      PURPOSE OF TRAINING
      This course involves hands-on manual therapy techniques including: soft tissue manipulation, joint mobilization (non-thrust), myofascial techniques, lymphatic techniques, and stretching. Students will both perform and receive these techniques as part of practical learning.

      ACKNOWLEDGEMENT OF PHYSICAL CONTACT
      I understand that this course requires direct physical contact between students and/or instructors. Techniques may involve contact with back, neck, shoulders, arms, hands, legs, feet, and abdomen (if applicable and consented separately).

      All contact will be educational, professional, and within scope of training.

      HEALTH DISCLOSURE
      I confirm I have disclosed relevant health conditions including recent injuries/surgeries, chronic pain, cardiovascular/neurological conditions, skin conditions/infections, or pregnancy. I understand failure to disclose may increase risk.

      RISKS & RESPONSIBILITIES
      I understand participation may involve mild discomfort, muscle sensitivity, or rare aggravation of pre-existing conditions. I agree to communicate discomfort immediately and always follow instructor guidance.

      VOLUNTARY PARTICIPATION
      I understand that participation is voluntary. I may refuse any technique or withdraw at any time without penalty. Alternative observation options may be provided where feasible.

      PROFESSIONAL CONDUCT
      I agree to maintain professional boundaries, treat peers with respect and dignity, and follow all academy policies regarding conduct and safety.

      SENSITIVE AREA CONSENT (OPTIONAL)
      I provide explicit consent for training involving:
      Abdominal techniques
      Chest/Pectoral region (draped and professional only)
      TMJ/Face techniques
